Peter Dobromylskyj of Hyperlipid has been looking at how different dietary fatty acids help or hinder mitochondrial control of cellular metabolism. He has two intriguing posts today:
– In the first, he proposes that saturated fats make long fasts more tolerable by enabling better control of energy utilization; polyunsaturated fats make long fasts intolerable and trigger hypoglycemia, stimulating appetite. This would be the second known pathway by which omega-6 fats upregulate appetite: omega-6 fats can also degrade to endocannibinoids which stimulate eating.
– In the second, he suggests that an omega-6 rich ketogenic diet would promote diabetes by creating conditions of cellular energy excess. Saturated fats protect against that scenario. Perhaps Peter and CarbSane can agree: There is such a thing as “nutty ketosis”!
Meanwhile, Jimmy Moore is doing well on nutritional ketosis, but Neely Quinn of Paleo Plan had trouble.
CarbSane finds another zero-carb danger: thiamin deficiency neuropathy.
Mark’s Daily Apple has a 204-page forum thread on the “Potato Diet.” Tatertot asked me what I thought of a potato-only diet as a weight loss tactic, and I gave my thoughts here. I might add: Safe starches have come a long way, if the hot new Paleo/Primal weight loss fad is 100% safe starch!

French fires made in grass fed tallow are my new health food and we have them 3 or 4 times a week! I wanted a way I could eat for life and I found it. FYI: I would guestimate I eat around 2300 calories a day. I am losing 2 pounds per week and am down to 372lbs.

I started a ketogenic diet for other reasons, but the first and most obvious change was that my migraines stopped. Hi Paul,
You may remember I commented about a month or so ago that I’d started following PHD after a year of struggling with debilitating hypothyroidism (despite supplementation with compounded thyroid and cytomel), along with low testosterone, low DHEA, underfunctioning adrenals, and a vitamin D deficiency. Less than two months ago, I was unable to drive, and was having trouble walking due to the fatigue and trouble with muscle coordination.
The results of my most recent blood work are in, and there are great improvements: my vitamin D levels have returned to optimal levels, my DHEA is normal, my testosterone has increased (although its not at a normal level yet), and my free T3 has increased to 3.1. Before, it was so low it wasn’t even on the chart and only my total t3 could be reported (which, obviously, was mostly reverse t3). My adrenals have yet to show any improvement.
I still have a way to go, but what a huge improvement! Most days, I feel pretty good. I’ll keep at it.

After reading “Perfect Health Diet” I increased my intake of saturated fats and reduced intake of grains and legumes. After a few months on the improved diet I had an echocardiogram and a VAP test. The echo showed that my ejection fraction had increased from 65 two years earlier to 75 which is about as good as it gets. The VAP test showed by TG dropped from 66 to 60 and HDL increased from 80 to 88. Apo B dropped from 98 to 90. Blood pressure is now 110 over 60.

Re: Kaleigh and her Leangains experience, there’s nothing definitionally ancestral or evolutionarily-appropriate about Leangains. It’s simply: fast for 14-16 hours a day; lift weights 3x/week; after lifting eat a lot of carbs and low fat; on rest days eat fewer calories and more fat.
There are no nuances to the carbs and fats. So without any sort of individual mandate to eat “clean” it’s easy to see it going the way of any diet’s Free Day: i.e., Twizzler-land.
I love my grossly modified, PHD-compliant Leangains. I lift once a week and have a massive Okinawan (purple) sweet potato feast for lunch afterward. Best day of the week!
Carbsane’s “thiamine deficiency in nutty-K” turns out to be a now-known side effect of the drugs the kids were also taking for epilepsy.
Not something that would have been fixed with a little white rice.
Epilepsy drugs tend to be very metabolically disruptive, so these case studies can produce some misleading ideas about ketosis.

That’s a pretty unconvincing case “Carbsane” is trying to make, especially given this from the study she cites:
“However, the ketogenic diets used in the treatment of childhood seizures and the low-carbohydrate/high-protein weight reduction diets are both thiamine deficient. Although thiamine requirements decrease as carbohydrate intake decreases, a thiamine deficiency may develop after long-term use of either of these diets.”
Yet beef and pork are two good sources of thiamine in the diet. How would one become thiamine deficient eating a high-protein diet consisting of beef and pork?
Also, the classic way to become thiamine deficient was by acquiring beri-beri: eating too much white rice.
I suspect the peculiar composition of the medical ketogenic diet is more likely the cause of this case report… Especially as this seems to be the only case report of thiamine deficiency on a ketogenic diet.

I can’t see how you could possibly come down with thiamine deficiency on a ketogenic Perfect Health Diet, for instance.
However, if you’re on a soybean-oil-based ketogenic diet, thiamine deficiency would be a real concern…
That post was pretty off the cuff. All manner of issues are reported in case studies in the literature vis a vis ketogenic (true, VLC) diets. In that study, the two subjects were not taking supplements, and had neuropathy reversed when thiamine was supplemented. One subject was taking meds, but he was still taking them all along throughout, so it’s a stretch to blame them George.
Yes, they needed thiamine, but this may have been just as much because of the drugs as because the diet was marginal.
These drugs have produced thiamine and biotin deficiency experimentally in subjects not in ketosis.
Whereas the sparing action of fat and protein on thiamine is actually a useful benefit of a high-fat diet.
Why didn’t this low-thiamine diet produce beri beri? Because it was a low-carbohydrate diet.
Although the second case doesn’t mention that drugs were taken during the diet, it’s possible that some were continued; the account doesn’t totally rule this out (the first case fails to mention the drugs or their cessation in the final status update, using the same words as the second case, so there is incomplete accounting for drugs by the authors).
The second case had in any case been exposed to these drugs and also periodically “refused to eat”.
This is probably not the usual low-carb scenario.
The authors only had one reference for anticonvulsants producing the symptom, from 1968, and no information about anticonvulsants and thiamine or biotin, as these interactions were only recognized and explored later.
In fact, papers like this might have contributed to later investigation of the antivitamin effects of anticonvulsants.

My favorite part of the Primal Chef video was seeing Stephan Guyenet comments as one of the judges. Plenty of rewarding food/flavors … his favorites were the spinach, bacon and squash, duck fat side dishes.

Here's the thing, a high carb/ low fat diet is going to do the same as DNL is going to produce saturated fatty acids (your omega 6 intake would already be really low on this type of diet).
It may still turn out that having your own body produce the fatty acids you need is quite beneficial in the grand picture. As you won't be consuming a lot of fatty acids that have been heated/manipulated/loaded with environmental toxins.
Consuming monounsaturated fats might also have a hormetic like fish oil because it forces your membranes to become a little less saturated. Who knows, this might even be the best scenario (just look at the longevity of those Sardinians. Once again, more research is needed.

Except that some of the glucose is metabolized for energy, not converted to saturated fat; and Peter has stated that the benefits of saturated fat come from driving complex II of the ETC not complex I, which glucose favors. So carbs wouldn’t be as good as saturated fat at this effect.

I agree what you say — the actual amount of DNL on really high carb diets is actually quite small, but the fat made would be saturated!!
I usually study things on the longevity angle and have come to almost the exact opposite opinion. When you are a baby, you are going to have less mitochondria as a percentage than when you are older. I believe that having too much muscle is also a negative to longevity because of the increase in mitochondria. I believe a body using the energy cycle for glucose will probably be safer in the long term than fat.
Once again — More studies needed.
Metabolic flexibility – even to a VLC believer, sometimes when eating out starch will be preferable to fat, if the fat with the meal is high in linoleate, and the starch is, say, rice.
I know Denise Minger had commented on her eyes becoming lighter as well. I think she speculated it was from increased glutathione intake.
